Clinical FAQs

When is a flat barrier appropriate vs. a convex barrier?

A flat barrier is appropriate when the stoma protrudes adequately (typically 1cm or more above the skin surface) and the surrounding peristomal skin is smooth and even. Convexity is indicated when the stoma is flush, retracted, or surrounded by skin folds, creases, or scar tissue that breaks the seal of a flat barrier. If a patient experiences chronic undermining or leakage despite a properly fitted flat barrier, convexity should be evaluated by a WOC nurse.

How do I measure for the correct pre-cut opening?

Use a Hollister Delma stoma sizing guide or equivalent. Place the guide over the stoma and select the smallest opening that fits around the stoma without touching or rubbing it. The opening should clear the stoma edge by no more than 1/8". Measure at each barrier change during the first 6–8 weeks post-op, as the stoma typically shrinks during this period. Once size stabilizes, a pre-cut barrier is appropriate for consistent reordering.

What is the Flextend adhesive and why is it preferred for ileostomy?

Flextend is Hollister's proprietary synthetic barrier material specifically engineered for high resistance to enzymatic erosion from liquid ileostomy output. Standard barriers can break down rapidly with constant exposure to ileostomy effluent; Flextend maintains integrity significantly longer, reducing change frequency and peristomal skin trauma. It is also suitable for urostomy, where consistent moisture exposure is the primary wear challenge.

What does the floating flange do clinically?

The floating flange is not rigidly fixed to the wafer surface — it has a slight lift that allows the user to slide fingers beneath it during pouch coupling. This reduces the direct downward pressure applied to the peristomal area when snapping the pouch onto the barrier. It is particularly beneficial for patients with abdominal tenderness, herniation, limited hand strength, or reduced dexterity.

Can the pouch be changed without removing the barrier?

Yes. The New Image two-piece system allows independent pouch removal and reattachment while the barrier remains on the skin. The patented locking mechanism holds the pouch securely with audible clicks during attachment and requires deliberate disengagement to remove. This allows the barrier to stay in place for its full extended wear period (typically 5–7 days) while the pouch is managed more frequently as needed.

Why is there no Yellow flange size shown in these SKUs?

Hollister's New Image system includes a Yellow 4" (102mm) flange size, but the pre-cut Flextend barriers are only offered in Green (44mm) and Red (57mm) in this SKU range. The Yellow flange is available in cut-to-fit format for patients requiring larger peristomal coverage. If you need the 102mm option, contact us to confirm current availability.

Are skin preps or barrier rings compatible with Flextend?

Hollister Skin Gel Wipes (#7917) and most skin prep wipes are not compatible with Flextend and will reduce wear time. Hollister Adapt barrier rings and paste can be used around the stoma opening for additional leak protection and are compatible with this barrier. Always allow any skin prep to fully dry before applying the barrier.
