Complete your purchase and we will send you a questionnaire. When we receive your completed questionnaire, we'll contact you to set up your appointment for the actual scan.

You will need to provide us with your:

2. Birthdate
3. Face picture with plain background.

The scan will be booked after receiving the questionnaire and picture back. The scan take about 15 minutes to do over the phone.
